What we will need from you

The first thing you will need to provide is your NHS number, this is often on your appointment letter.

You will also need to give the details of your GP surgery and the address of where you will be picked up from and the hospital your treatment is in. Make sure to tell EEAST when you book if you have any specific requirements so that the most suitable form of transport is provided for you.

Keep a note of your unique booking reference number when this is provided to you in case you need to get in contact about your journey again. If your appointment gets cancelled, changed or you no longer need transport please call the number above again to let them know as soon as possible.
